Reminder: Physical Exams Needed for Fourth Graders
If you have not done so already, please remember to provide a copy of your 4th grader's current physical examination to the nurse's office. The physical must be from within the last 12 months and include the health care provider’s signature and the date of examination. Please send this in at your earliest convenience.
If your student has not had a physical within the last 12 months, kindly send me a confirmation of their upcoming appointment and provide a copy upon completion.
Physicals can be uploaded into the SNAP Health Portal, faxed, or mailed to me.
PS: If you have a student outside of 4th grade, but would still like to provide a copy of their latest physical, I will happily accept! It is always great to have students' most current health records on file.

If your family is in need of support, please complete the APS Family Support Request Form. Once you complete this form we will share your responses with the family engagement department and/or school social workers or counselors to support your family.
